HEADCHECK Forms is an add-on that was designed to help organizations meet their requirements for general waivers and education files (including videos) that you want your athletes, (and their parents/guardians, if applicable), to review and sign. Basically, it allows an organization administrator or staff member to:
- Upload waivers and files for your organization or for a specific team
- Allow each athlete (or their parent/guardian) in your organization/team to review and sign the file in HEADCHECK
- Quickly see which athletes have the file signed and which haven't
- Quickly send an email reminder to athletes (or their parents/guardians) to review and sign the file
We originally designed this specifically to help organizations comply with their Concussion legislation (e.g. Rowan's Law - Ontario).
