It’s important to make sure you’re following your organization’s and your region’s COVID-19 protocols. To help with this, we added a COVID-19 screening tool to HEADCHECK Health. This includes a COVID-19 screening questionnaire for you to fill out before each game and practice, as well as automatic notifications sent to your team’s medical staff so they can follow up with next steps if needed.
Check with your team’s medical staff member to see if your team is using this tool. If yes, then continue reading below to see how to complete and review your COVID-19 Symptom Screen using HEADCHECK.
There are three ways to complete the questionnaire:
- Your team’s medical staff member can login to their HEADCHECK account and help you complete the questionnaire
- You can do it yourself using your HEADCHECK Athlete account
- For minors, a parent/legal guardian can complete the questionnaire for the athlete using their HEADCHECK Guardian account
