It’s important to make sure you’re following your organization’s and your region’s COVID-19 protocols. To help with this, we added a COVID-19 screening tool to HEADCHECK Health. This includes a COVID-19 screening questionnaire to fill out for your child before each game and practice, as well as automatic notifications sent to your child’s team medical staff so they can follow up with next steps if needed.
Check with your child’s team’s medical staff member to see if your child’s team is using this tool. If yes, then continue reading below to see how to complete and review your child’s COVID-19 Symptom Screen using HEADCHECK.
There are three ways to complete this COVID-19 Symptom questionnaire:
- The athlete’s team medical staff member can login to their HEADCHECK account and help them complete the questionnaire
- The athlete can do it him/herself using their HEADCHECK Athlete account
- For minors, a parent/legal guardian can complete the questionnaire for the athlete using their HEADCHECK Guardian account
